One or more of the 2142 op-amp IC chips is more than likely bad. Failed IC chips may be hot to the touch. Refer to the pdf below:

Connect with the Surface specific GUI and make sure there is a Blade ID and IP address both under the Mix Engine's Blade Info

Ensure your surface is added as a peripheral device in Navigator. If it is, make sure the crosspoints are made to the correct hdpn and cue destinations.

Check status led on power supply to ensure the supply is on. Ensure power supply cable is seated correctly and securely. Confirm the dc cable going to the console has not come lose or fallen out.

Check IP settings, swap your cable, or try a different switch port. Check PC firewall and anti-virus software. Plug directly into the device from your PC and attempt to ping it's IP address to rule out a network issue.

Our support team can remote into your local PC and reload software in the panel host cards. Only send in the effected panel host card mounted in the LXE. There is no need to send the entire surface for a panel host issue.
